Fix Reviewer and Metareviewer Mappings (Issue711)
Test Example
1. Log in as instructor6
2. Select "Manage Assignments" then click on the "Assign review" link for "Design exercise" assignment because it has a review strategy of "Instructor-Selected"
3. Click on Import reviewer mappings
4. Create an import file 1.csv with the Contributor and reviewers. The content of our 1.csv is like
